We want to help you get your pet fixed (spayed or neutered). If you get help from the government or don’t make a lot of money, we may be able to help pay for it.

How to Ask for Help

1. Call or email us:
   Call 541-200-2525 or email scheduling@fotas.org
   If we don’t answer, leave a message. We’ll call you back in about 3 days.
2. Tell us about your family and money situation.
   We’ll ask a few questions to see if you qualify.
3. Send your papers:
   Email your ID and proof of income or assistance to scheduling@fotas.org OR bring copies to us before your pet’s appointment.
   You must send these within 3 days after we say yes.
4. Pay a $25 fee:
   This helps us run the program. It is not refundable.
5. Please note:
   We don’t have help for everyone. Money goes fast. You must apply early.
   We do not keep a waitlist.
   You must be approved before your pet’s surgery day.
   We can’t accept papers the day of surgery.

What Papers You Need to Show

Send one of these:

• A letter with your name showing you get:
  • SNAP (food stamps)
  • TANF (cash help)
  • Section 8 (housing help)

• A letter showing you get:
  • SSDI or SSI (Social Security retirement doesn’t count)

• Your most recent 1040 tax form
• Two pay stubs
• A benefits letter (like unemployment or VA)

• Dogs: Must be on a leash or in a carrier
• Cats: Must be in a carrier (one cat per carrier)
